Welcome to the Prospect members' survey. Below you will find instructions on how to complete the survey and how to enter the prize draw. The survey is comprised of a series of multiple choice questions, most questions ask for a single response. Some questions will give an option for more than one answer or a written response. Please follow the guidelines when answering these questions. The survey should take no longer than 15 minutes to complete.

To orientate through the survey answer as many questions as you are able and use the buttons at the bottom of the page to navigate not the back button on the browser. To move forward click the 'next' button, to return to a question on the previous page click 'back', to change your answers on that page click 'reset' and click 'submit' on the final page to complete the survey.

The survey will close on Friday 19 February 2010. To be eligible for the prize draw you must provide a contact email address or telephone number at the end of the survey. This information will not be used to analyse the survey results, all responses will be held in confidence. Thank you for taking the time to give us your views.
